Terms and Conditions of Service
These Terms and Conditions of Service ("Agreement") govern the wireless telecommunications and Lifeline-supported broadband services provided by the selected Lifeline service provider.
By applying for, enrolling in, activating, or using services through this enrollment portal, you acknowledge that you have read, understood, and agree to these Terms and Conditions.
This enrollment portal may be operated by Medequip Mobile as an authorized enrollment and marketing partner for the selected Lifeline service provider. Wireless and Lifeline services are provided solely by the selected Lifeline service provider.
The selected Lifeline service provider reserves the right to modify these Terms and Conditions at any time.
Lifeline Program Terms
The Lifeline program is a federal assistance program supported by the Universal Service Fund and administered by the Universal Service Administrative Company ("USAC"). Eligibility determinations are made through the National Verifier system administered by USAC.
Only eligible consumers may enroll in Lifeline service. Eligibility is determined based on participation in qualifying federal or state assistance programs or by meeting applicable income requirements.
Federal rules limit Lifeline support to:
- one benefit per eligible household
- either wireline or wireless service, but not both
A household is defined as any individual or group of individuals living together at the same address who share income and expenses.
By applying for Lifeline service, you certify that:
- the information provided is true and accurate
- you are eligible for the Lifeline benefit
- no other member of your household currently receives a Lifeline benefit unless permitted under FCC rules
- you will notify the selected Lifeline service provider within 30 days if you no longer qualify
- you consent to eligibility verification through USAC and applicable systems
- you understand that providing false or fraudulent information is punishable by law
Customers must complete annual recertification as required by FCC and USAC rules. Failure to recertify eligibility may result in de-enrollment from the Lifeline program.
Enrollment Partner Disclosure
This portal is operated by Medequip Mobile as an authorized marketing and enrollment partner for the selected Lifeline service provider.
Application information submitted through this portal may be collected by the Enrollment Partner and shared with the selected Lifeline service provider solely for purposes of eligibility verification, enrollment processing, customer support, fraud prevention, and Lifeline service administration.
Consent to Communications (TCPA)
By submitting an application or providing your contact information, you expressly consent to receive calls and text messages from the selected Lifeline service provider and its authorized representatives regarding:
- enrollment status
- account notifications
- recertification reminders
- service-related communications
- customer support
These communications may be made using automated dialing systems or prerecorded messages where permitted by law. Consent is not a condition of purchasing any goods or services. Message and data rates may apply.
Electronic Signature & Records Consent
By submitting your application electronically, you consent to:
- use of electronic records
- electronic signatures
- receiving required disclosures electronically
You agree that your electronic signature has the same legal effect as a handwritten signature.
Activation and Service
Telephone numbers are assigned at activation. Customers do not obtain ownership rights to assigned numbers. Wireless service may be provided through third-party network operators and coverage availability may vary.
The selected Lifeline service provider may modify services, update software, suspend or terminate service, and change plans or features without prior notice where permitted by law or required for operational, regulatory, or fraud-prevention purposes.
Non-Usage & De-Enrollment
FCC rules require active usage of Lifeline-supported service. If no qualifying usage occurs for thirty (30) consecutive days, notice of pending de-enrollment may be issued.
Qualifying usage may include:
- making or answering calls
- sending text messages
- using data
- purchasing airtime
- responding to communications indicating continued intent to use service
Failure to establish usage after notice may result in Lifeline de-enrollment and/or service deactivation.
Fraud Prevention
Lifeline-supported devices and services are intended solely for use by the enrolled subscriber. Selling, transferring, or misusing Lifeline-supported devices or services may violate federal or state law and may result in de-enrollment, service termination, account suspension, or referral to appropriate authorities.
Limitation of Liability
The selected Lifeline service provider is not responsible for service interruptions, network availability, delays, dropped calls, data loss, or unauthorized access to the maximum extent permitted by law.
